Your medical insurance may cover the treatment for Sleep Apnea and TMJ issues, and our office will do a verification of benefits from your insurance (including Medicare and the Veterans Health Administration (VHA)). If you are consulting with Dr. Chitgopekar on these, please fill out and print the following forms before your first visit.
